The insurance industry is adopting AI faster than it is building the guardrails to govern it. And that gap is where risk lives.

AI can now perform work equivalent to roughly 12% of U.S. jobs, representing approximately $1.2 trillion in wages. For insurers, claims processing, document review, and fraud detection are among the first functions to feel the impact. But speed without structure creates exposure. At a recent Insurance Thought Leadership webinar, our Head of Machine Learning Denys Linkov joined Les Shute of Kairos Kinetics and moderator Paul Carroll to discuss what responsible AI governance actually looks like in practice, not in theory.

The first mistake organizations make is treating all AI as one thing. As Les Shute put it during the session: "AI is nothing new. Generative AI is new."

Traditional AI, the kind that has been powering fraud detection models and claims triage for years, operates on structured rules and trained patterns. It is predictable. It is auditable. Most insurers already use it in some capacity, with 84% of health insurers across 16 states reporting AI or machine learning usage.

Generative AI is fundamentally different. It creates new outputs from learned patterns. It can draft summaries, analyze unstructured documents, and extract insights from thousands of pages of medical records. But it can also hallucinate, fabricate citations, and produce confident-sounding errors.

This distinction matters because your governance framework needs different controls for each. A fraud detection model needs bias testing and outcome monitoring. A generative AI document summarizer needs accuracy validation, source verification, and expert review loops.

Why Human-in-the-Loop Is Non-Negotiable

The data on this is unambiguous. According to a joint survey by Wisedocs and PropertyCasualty360, only 16% of claims professionals trust AI output on its own. But when human expert validation is part of the process, trust jumps to 60%, a nearly 4x increase.

That is not a soft preference. That is the market telling you: autonomous AI will not be adopted at scale in claims. Human oversight is the trust mechanism that enables deployment.

"Human judgment is particularly important when it comes to things that affect humans," Shute noted during the webinar. In claims, every AI-generated summary, every flagged document, every suggested outcome touches someone's livelihood, their medical care, their financial stability.

The expert-in-the-loop model works like this:

  1. AI handles the heavy lifting. Document ingestion, deduplication, structuring, timeline creation. In one case, 60% of 20,000+ processed pages were found to be duplicates or irrelevant material. AI identifies and removes those instantly.
  2. Domain experts validate. Clinical specialists, claims professionals, or legal reviewers check AI outputs before they reach the decision-maker.
  3. The decision-maker acts on verified intelligence. They receive organized, validated, defensible information rather than raw AI output.

This is not slower. Organizations using this approach report 59% faster case processing, 33% administrative cost reductions, and 63% improvements in customer satisfaction. The human layer does not create a bottleneck. It creates accountability.

Building Your Governance Framework: Five Practical Steps

Based on the webinar discussion and real-world implementation experience, here is a governance framework that works for claims organizations at any stage of AI adoption.

1. Establish a Cross-Functional Governance Council

AI governance cannot live in IT alone. Your council should include representatives from claims operations, legal and compliance, information security, clinical or medical review (if applicable), and executive leadership.

This council owns the policies, reviews vendor relationships, and audits AI performance. Without cross-functional input, governance becomes either too technical to enforce or too vague to be useful.

2. Demand Vendor Transparency

"The biggest risk is taking something shiny and putting it in the forefront of your technological output," Linkov cautioned during the session.

Before deploying any AI system, require vendors to disclose: - How they handle your data (storage, access, retention, deletion) - Their model architecture (is it a fine-tuned model or an API wrapper?) - Their security posture (SOC 2 compliance, encryption standards, access controls) - Their approach to bias testing and accuracy measurement - Whether they use your data to train their models

If a vendor cannot answer these questions clearly, that is your answer.

3. Apply Cloud Security Lessons

The insurance industry spent two decades learning cloud security the hard way. Apply those lessons to AI deployment now: - Single sign-on and multi-factor authentication for all AI platforms - Role-based access controls (not everyone needs access to raw AI outputs) - Audit logs for all AI-generated recommendations - Data classification standards that extend to AI training and inference data

4. Separate Automation from Augmentation

Not every process should be fully automated. Map your AI use cases on a spectrum:

Document deduplication Medical record summarization
Page classification Claims outcome recommendations
Data extraction from structured forms Fraud flag validation
Record indexing Complex case triage

Automate the mechanical. Augment the judgmental. This is not a philosophical position; it is a risk management decision.

5. Build Governance Before Deployment

The most common mistake is deploying first and governing later. By the time you discover a bias in your claims triage model or an accuracy issue in your document summarizer, the exposure has already accumulated.

Define your acceptable accuracy thresholds, your escalation procedures for AI errors, and your audit cadence before the system goes live. Integrate security and legal teams into the development process, not just the review process.

The Regulatory Landscape Is Moving

California's SB 574 and similar state-level legislation signal that AI governance in insurance is shifting from voluntary best practice to regulatory requirement. Organizations that build governance frameworks now will have a compliance advantage. Those that wait will face retrofitting costs, potential penalties, and the much harder task of adding controls to systems already in production.

What This Means for Your Claims Organization

The insurance industry does not need to choose between AI adoption and responsible governance. The organizations seeing the best results, 70% reductions in claim review time, 95% accuracy maintenance, measurable ROI, are the ones that treat governance as an enabler, not a constraint.

Start with a governance council. Audit your current AI vendors. Map your processes on the automation-augmentation spectrum. And build human oversight into every workflow that touches a claimant's outcome.
